4. Send the completed form to the ETS registrar through the U.S. Postal Service, please see the ETS Board Members Page for current address informaton for the registrar. The address of the registrar is also found on the refund form but may be out of date. You are encouraged to retrieve the address from the ETS Board Members Page .
