2. Select the writeable pdf file link below
   
3. Complete the top section of the form using you computer.
4. Save it locally by using the "Save"    button on the tool bar, giving it a unique name
   e.g. refund_request
   
5. Send completed form to the ETS registrar by email,  registrar@eagansoccer.org or through the U.S. Postal Service.  Please see the current Board Members Page for current address information.
